Appointments
We are currently accepting patients at all locations. Our patients come to us by a number of routes including, "word-of-mouth" referral, as well as direct referral from the patient's primary care or Ob/Gyn physician. For patient convenience, we have 2 offices which are listed above. We accept most insurance plans and patients can check with our receptionist when making an appointment regarding coverage.
It is recommended that new patients bring copies of their prior treatment records or have them forwarded to us before the initial consultation. If this is not possible, patients can sign a record-release form at the time of the first visit. If at all possible, we would encourage the male partner also be present at the initial consultation. The time of the initial visit is usually 45-60 minutes.
Our goal is to provide outstanding patient care by individualizing each patient/couple when determining evaluation/treatment plans. For more information, please call (513) 585-2355.
